Card application for schools

Dear Principal,

We would like to send your school a new Des Plaines Public Library card for the 2025 - 26 school year. Please note that your current card will expire on October 1st. Please authorize one staff member as the designated school representative who will familiarize themselves with the privileges of the card and supervise its use among other staff members.  Kindly note that all materials checked out are the responsibility of the school, including reimbursement if items are lost.

Please return one signed copy of this letter.  Upon receiving this signed form, we will send you the new card.

If the school library card should become lost at any time, please notify the library immediately. Thank you!
